I definitely remember the details and there is no excuse for poor decision making behind the yoke. Sometimes there just isn't much margin for error though even in the best of decision making scenarios.

There are way more cold calls on ground then any other role in the system. The exchanges also tend to be longer. I used to, up until recently, be able to change my default device to my headset in the Windows control panel, start Pilotedge, then reselect my device to my speakers in Windows.
