For anyone that still uses Servinfo, I created airline profiles and logos for ExpressJet Airlines, Spirit Airlines, and Virgin America. To add these profiles first BACKUP your servinfo.dat file. Once that is completed, open servinfo.dat using notepad. Add these lines to the very bottom of the text.....

This is a common area of confusion for most pilots. Visual approaches have no published missed. As Anthony said its pretty much up to the tower controller. For reference it is explained in the AIM section 5-4-23.
