Transition When Passing VOR and/or Executing SIDs and STARS

I have tried a number of ways to make the passing over VORs a graceful transition. Unfortunately, it still *feels* jerky and uncoordinated. I am now pursuing my I-5, but I am somewhat concerned about following the SIDs/STARs with any kind of elegance unless I can first handle a simple VOR transition with coordinated confidence.

Please tell how you make your transitions?
