Odd plane behavior and crash

So I finally got everything working and set off for my I-1 rating. Got clearance, then taxi to the runway and cleared for takeoff. Then as I take off, the plane does some crazy spin to the left and I crash. Never done anything like that before. So I disconnected and reset on the same runway and powered up and took off with no problem. Any ideas on why this could occur?

do you have your random failures turned on?

I had this happen once and only once (but it was so memorable I'll never forget it). It turned out I had an axis or key, not sure which, bound to aileron trim, and bumped it enough to crank it all the way over when setting up and of course it wasn't relevant until the plane was in the air. Might check your key and axis assignments and see if something similar happened.
