"... from the marker"
If I hear something like "123AB 5 from the marker ...", does that mean 5nm from the OUTER marker (I know what the OM, MM and IM are, just not sure about the phraseology ...)?

A controller would only correlate your distance to a fix on the final approach course of the approach procedure. Whatever the marker shown on the chart would most likely be what they're referring to.

This will, by process of elimination, be the outer marker...100% of the time. The middle and inner markers, if present, are well inside the FAF, and since ATC is vectoring you well outside the FAF, the IM and MM are not relevant.
