Incident Report Thread

Postby Kilstorm » Thu Nov 21, 2019 7:00 am

Almost all of my flights are done with planes or addons that give me a chance to have to deal with something wrong happening. For GA, I fly A2A planes in P3D and REP planes in Xplane so both of those have the plane age and in doing so there is a chance things might break in mid-flight. I also fly two commuter planes using FSPassengers with a 5% failure rate. I'd prefer not to have anything go wrong but always knowing there is that chance and then when it does having to deal with it for practice in case that happens in real life is part of the sim to me.

I hate to declare an emergency, I dont like the idea of taking away a controllers time from other pilots they are working with and I dont want to the controllers to think that I am deliberately forcing something to happen making me a priority and distract from their duties. I do my walk arounds, look at my maintenance reports, do repairs when needed. So I thought it would be nice to have a dedicated forum thread in the pilots forum to where pilots like me could post a report of what happened post flight. A thread ATC could reference as a place for pilots to go and make their report after the pilot has landed. Very much like what a pilot would need to do with the FAA.

No thats more for newbie mistakes. I'm talking about being able to post a report of what happened. As an example, I landed in Aspen and was rolling down the runway coming to a normal stop. All of a sudden the plane veered off the runway at about 40kts slowing. I came to rest off the runway. So when I pulled up the report I found that yes I hand made a nice landing at 106kt, in a 49 feet per minute decent with a 5 degree pitch so textbook landing but blew a tire. Something that I couldnt see upon departure was a low tire. Heres the full report from FSPassengers.

Incident Report:

The plane took off with some tires under-inflated and one tire exploded when landing. The pilot was able to land the aircraft safely.

Feel free to start your own thread for it! Just give people a template or something to work off of so the expectations are set from the beginning.
