When at a class D towered airport and instructed to make "left pattern for runway 3," am I supposed to wait for the base leg turn until tower clears me for landing? This seems to be the norm, but I can't find anything in the FAR/AIM to indicate what is acceptable.

For whatever reason, "Radar flight following unavailable." I was confused, but continued VFR at that point.

When going for my V-02 two "odd" things happened which I was unsure of.

So, after taking a closer look a the charts, I have a couple of theories:
(1) Class B & C airports have a note to "Contact XXX Approach within 20nm" in the charts. I suppose this is the cue to contact approach and not tower.
(2) No approach contact needed for class D airports.
