San Carlos departures

Gentlemen, can you help me with interpretation of DP for SQL?
Take off Minimums:
SAN CARLOS, CA
SAN CARLOS (SQL)
AMDT 1 09295 (FAA)
TAKE-OFF MINIMUMS: Rwy 30, NA.
DEPARTURE PROCEDURE: Rwy 12, climbing left turn
via heading 120° and SJC VOR/DME R-281 to SJC
VOR/DME before proceeding on course.
Keith explained to me that when flying IFR, I will most likely get the clearance like :" Fly runway heading until passing the diamond shaped waterway, then turn right heading 120° within 2 miles of the airport for vector to the assigned fix/route, maintain VFR at or below 1,100 until passing the OAK 165° radial, maintain 2,000 feet, and to expect assigned altitude 5 minutes after departure." ( I did get in just like that)
1. Why in the Take off Minimums, Rwy 30 is not applicable?
2. What happen if I have to take off from runway 30 due to the winds?
3. When would DEPARTURE PROCEDURE: Rwy 12 apply and how is that related to Tower hours?
4. Withing the Tower operating hours, I would get VFR or IFR take off instructions, correct?
5. If the Tower is closed , how would I depart?

How would I fly it height wise, there is class D PAO and class C SJC?
