Four basic radio phraseology questions

Four really basic and quick questions:
(1) When I get 'frequency change approved' from the controller, what do I respond? Just 'roger'?
(2) Leaving a class D airport into uncontrolled airspace: If the controller does not come back to me with 'frequency change approved', may I leave the frequency anyway, or do I need go inform the controller (with something like 'request frequency change')?
(3) When I get wind information from the controller when on the final, what do I respond? Just 'roger'? (I know I don't have to readback the information at least.)
(4) After landing at a controlled airport I leave the runway, and so far I have been contacted automatically by the Tower with taxi instructions. But if they forget me, shall I just switch to Ground and ask for instructions (without saying "goodbye" to the Tower first)?
