VOR Approach Question

I'm looking at the VOR approach for KCMA: http://flightaware.com/resources/airpor ... RWY+26/pdf

If I fly outbound from VNY VOR, there is a hold indicated at COOGA. Can someone explain the procedure here? Is the hold mandatory or only if directed by ATC? If the hold is mandatory, do I remain in the holding pattern until cleared by ATC for the rest of the approach, or just one circuit then proceed with the rest of the approach?

If you see the words "No PT" that means no procedure turn

If you start from VNY you do not perform the hold

Someone did post a similar question, but it was another approach and it's easy to imagine that someone might have missed that thread.

Caesar, it's a detailed topic, I would highly recommend watching the workshops on approaches.
