I think I first found out about the NASA Aviation Safety Reporting System newsletter "Callback" from a post on the forums (current issue) and it's proved to be a great source for "think before you fly" stuff. This month it's fuel-related and it's amazing how folks have made what seems like an obvious mistake but out of character.
